Allows submissions of certain forms over the web, such as, Address Changes, Enrollment Forms, Termination Forms, ID Card Requests, etc. This also provides access to PDF forms that can be downloaded for fax or mail.

** The screen samples shown in this help system are just examples and the forms or options you see may differ. Each client group can customize the options available. The forms listed will vary from user to user.

Click on forms to fill out information on the data entry screen. Each form will vary in appearance and required fields.

Fill out the form data.
Some forms allow for file attachments. Only allowed file types can be uploaded with the form, and some disallowed special characters in the file name will be removed during the file upload.
Click SAVE.
