Allows submissions of certain forms over the web, such as, Address Changes, Enrollment Forms, Termination Forms, ID Card Requests. This also provides access to PDF forms that can be downloaded for fax or mail. The forms listed will vary from user to user.

  1. Log into Web eXchange Member Center

  2. Select the “Forms" button from the toolbar

 

 

Click on forms to fill out information on the data entry screen. Each form will vary in appearance and required fields.
