This screen allows the online submission of certain forms and questions, and also provides access to PDF forms that can be downloaded for fax or mail submission.

Questions and Web Forms
Click on forms to fill out information on the data entry screen. Each form will vary in appearance and required fields.
Some forms allow for file attachments. Only allowed file types can be uploaded with the form, and some disallowed special characters in the file name will be removed during the file upload.
Click SAVE to submit the form/question.
A transaction ID will be displayed for tracking. All previously submitted transactions (questions and forms) can be viewed from the My Account\My Transactions main menu option.
Submitted forms/questions will be reviewed/processed by HealthSmart Network Solutions teams and the transaction will be updated with information as it is processed.
