Allows submissions of certain forms over the web, such as, Address Changes, Enrollment Forms, Termination Forms, and ID Card Requests. This screen also provides access to PDF forms that can be downloaded for fax or mail.

- Click on forms to fill out information on the data entry screen. Each form will vary in appearance and required fields.
- Search for, then select a participant that the form is in relation to (if applicable to the form).
- Fill out the form data.
- Click SAVE.
