Form Count By Client

Use this report to show all forms submitted from all clients in a date range or for specific clients in a date range.  The report will show forms submitted by members, clients or providers as long as the form submitted was in reference to a member that is tied to a an employer group.



  • Form Completed Date From:  Enter the date the report should start from.  This date refers to the date the form was originally submitted.
  • Form Completed Date To:  Enter the date the report should run through.  This date refers to the date the form was originally submitted.
  • Employer:  Enter the employer(s) the report should be restricted to.  Leave empty for all employers.
  • Report Format:  Select PDF or Excel for the output format of the report.
