Allows submissions of certain forms over the web, such as Precertification Requests and Claim Forms. This screen also provides access to PDF forms that can be downloaded for fax or mail.
Forms shows in this help system are only for example and may not be available in the provider center.
Click on forms to fill out information on the data entry screen. Each form will vary in appearance and required fields.
- Search for, then select a participant that the form is in relation to (if applicable to the form).
- Fill out the form data.
- Some forms allow for file attachments. Only allowed file types can be uploaded with the form, and some disallowed special characters in the file name will be removed during the file upload.
- Click SAVE.
